&lt;p&gt;Target Audience:&lt;/p&gt;

&lt;p&gt;This program is intended for PCPs, including physicians, PAs and NPs, and other secondary audiences, such as geriatricians, geriatric psychiatrists, psychiatrists, and OB/GYNs, who may be involved in the care of patients with cognitive decline due to early AD.&lt;/p&gt;

&lt;p&gt;Learning Objectives&lt;/p&gt;

&lt;p&gt;Upon completion of this activity, learners should be able to:&lt;/p&gt;

&lt;p&gt;● Differentiate normal aging from mild cognitive impairment and early symptomatic AD using validated cognitive assessment tools and longitudinal clinical indicators&lt;/p&gt;

&lt;p&gt;● Implement evidence-based and guideline-compliant care strategies for patients who may be in the early stages of AD, including administering referrals to specialists as appropriate&lt;/p&gt;

&lt;p&gt;● Apply patient-centered communication and shared decision-making strategies to engage patients and care partners in discussions about cognitive concerns, diagnostic evaluation, and next steps in care considering the multidisciplinary team&lt;/p&gt;

&lt;p&gt;&lt;br&gt;&lt;/p&gt;

&lt;p align="center"&gt;&lt;br&gt;&lt;/p&gt;
