Target Audience:
This program is intended for PCPs, including physicians, PAs and NPs, and other secondary audiences, such as geriatricians, geriatric psychiatrists, psychiatrists, and OB/GYNs, who may be involved in the care of patients with cognitive decline due to early AD.

● Differentiate normal aging from mild cognitive impairment and early symptomatic AD using validated cognitive assessment tools and longitudinal clinical indicators
● Implement evidence-based and guideline-compliant care strategies for patients who may be in the early stages of AD, including administering referrals to specialists as appropriate
● Apply patient-centered communication and shared decision-making strategies to engage patients and care partners in discussions about cognitive concerns, diagnostic evaluation, and next steps in care considering the multidisciplinary team
